Legal Requirements
There are a few legal requirements that must be met before the state will issue a registry card. The applicant must be eighteen years of age or older, and a legal resident of the state of Illinois. In addition, a registered physician must have diagnosed the patient with a serious and severe medical condition. This physician must also be willing to certify their diagnosis along with their treatment recommendation. The patient must also have registry identification card, meaning he or she needs to pay the fee and complete the application process before the treatment can be administered.
Application Process
There are a number of different forms that may need to be filled out throughout the application process, which can make it confusing to many patients. In simple terms, patients will need a Physician Certification Form completed by their doctor, within ninety days of their appointment. This form must be submitted to the Department of Public Health along with the application fee and the Qualifying Patient Application Form and the Fingerprint Consent Form. In some cases, a third party may need to pick up your medication on your behalf. If this is the case, you will also have to complete a Caregiver Application Form. Patients are also required to send a few additional documents to the Department of Public Health along with their application. Proof of residency, in the form of two different documents with corresponding addresses, and proof of age and identify are also required.
